Bayesian analysis for child maltreatment in Peruvian adolescents | Author : Jonatan Baños-Chaparro | Abstract | Full Text | Abstract :Introduction: child maltreatment is a common psychological problem of children and adolescents around the world. Presence of various form of abuse can lead to other psychological and physical problems, so, it is necessary to identify which form of abuse is more likely to affect men and women. Objective: to analyze the differences observed in child maltreatment according to sex in Peruvian adolescents. Method: a cross-sectional, and quantitative and comparative approach study was carried out in the months of July and August 2022. A convenience sample of 1.376 adolescents who responded to a self-report on child abuse was analyzed. To compare both groups (men and women), the Mann-Whitney U-test for Bayesian independent samples was used. Results: among the various forms of child maltreatment, only was found in favor and with high evidence as alternative hypothesis, the emotional maltreatment (BF10 = 190.38; d = 0.31; 95 % CI: 0.20-0.48). This difference was higher in females (M=8.28) compared to males (M=7.09). In emotional and physical neglect, strong evidence was reported for the null hypothesis. Robust Bayes factor analysis confirmed the same results. Conclusions: emotional maltreatment occurs mostly in adolescent females, being the gender that requires more psychological support. In the other forms of maltreatment, no differences were found in correspondence with gender. Consequently, health professionals should consider the role of gender in emotional maltreatment, but should not neglect the other forms of child maltreatment, emphasizing the specific particularities of each case. |
| Management of the health care system in Ecuador to confront the antimicrobial resistance | Author : María José Goyes-Baca, Melanie Raquel Sacon-Espinoza, Francisco Xavier Poveda-Paredes | Abstract | Full Text | Abstract :Introduction: the Health Care System in Ecuador has a National Plan for the Prevention and Control of antimicrobial resistance, which aims to improve the quality of medical care and based from this goal focus attention on the increase in microbial resistance, which has become a highly relevant problem for global public health. Objective: to systematize knowledge on how the Ecuatorian health care system confront the antimicrobial resistance. Method: an exhaustive bibliographic review was carried out in bibliographic search engines such as Scopus, Google Scholar and PubMed, with the use in the search process of expecifics keywords. The research was carried out on May 20, 2022, and of the 39 articles only 20 were selected according to the criteria needed. Results: Ecuador has a mixed health care system, made up by the Ministry of Public Health (MSP), the Ecuadorian Social Security Institute (IESS), the Social Security of the Armed Forces (ISSFA) and the Institute Security National Police (ISSPOL), all representing the public sector; on the other hand, representing the private sector, it´s included the National Association of Private Clinics and Hospitals of Ecuador (Asociación Nacional de Clínicas y Hospitales Privados del Ecuador). Both are integrated to guarantee the integral health development in the entire Ecuadorian population, regulate and control the activities conducted by both public and private entities, without distinction, and to face the threat posed by the increase in the indiscriminate use of antimicrobials and its consequent repercussions on human and animal health. Conclusions: the inefficient way of combating the health risk related to antimicrobial resistance affects the Ecuadorian population due to indiscriminate and inadequate use of antibiotics and the insufficient use of conventional treatments. |
| Precondiciones clínicas y ecocardiográficas en la enfermedad obstructiva de tronco coronario izquierdo | Author : Yoandro Rosabal-García,Marilaicy Duconger-Danger, Lorchen Torres-Quiñones | Abstract | Full Text | Abstract :Introduction: cardiovascular pathologies are currentely the leading cause of death in the world, of which multivessel disease provides a high number of patients, with a marked annual mortality rate higher than those with single-vessel disease. Objective: to relate clinical and echocardiographic factors associated with left main coronary artery disease in patients with acute myocardial infarction at the Centro de Cirugía Cardiovascular y Cardiología in Santiago de Cuba, Cuba. Method: a prospective, analytical, non-experimental case-control study was conducted (case group 100 patients; control group 300 patients), during the period marked from 2017-2021. It were applied tests of the Kolmogorov-Smirnov method and, the odds ratio (OR) and the Durbin-Watson test were calculated. Results: the prevalence of acute myocardial infarction with ST segment elevation was found in 326 patients (81.5%) with a value of p = 0.05 (0.048); the presence of complications had a very high statistical significance with p = 0.05 (0.00714) as related value; the parietal motility index ?1.7 points showed a value of p = 0.05 (0.006) with an OR of 2.063; LI 1.229 and LS 3.463. The equation predicted at a 59.7% the risk of acquiring a multivessel disease in patients with: ejection fraction less than 50% in left ventricular diameter >58 mm, left atrial pressure >25 mmHg, PMAP > 15 mmHg and decreased right ventricular systolic function. Conclusions: the presence of complications, the type of acute myocardial infarction, and echocardiographic parameters such as parietal motility index were predictors of multivessel disease present in the acute events. |

Objective: to assess how a writing program aimed at writing clinical histories favors written communicative competence in students of Dentistry at the Universidad de Ciencias Médicas in Camagüey. Method: a pre-experimental study was carried out from March to December 2020 in students of dentistry career at the Clínica Estomatológica Provincial Docente "Ismael Clark y Mascaró", located in the faculty with the same name. The population studied involved a total of 101 fifth year students and 30 of them were selected as sample. The method of interview was conducted on 10 professors. The study took place in three moments: Diagnosis, Intervention and Evaluation. The workshop procedure was chosen as the organizational form. Descriptive statistics were used for data processing. Results: the application of rules, coherence, cohesion and the interviews quality were affected by the students unfamiliar practice on the writing process and because the short time of procedures limits reading and correction. Conclusions: assess of the program proposed has made the possibility of getting to know the difficulties presented by the students. It promotes knowledge and awareness of the processes involved in writing and giving respect for each recursive moments through which writing takes place. |

Objective: to determine dimensions and attributes to get internal customer satisfaction and loyalty in health institutions in Barranquilla, throughout 2021. Method: an exploratory descriptive study was carried out in healthcare facilities in Barranquilla. The study was conducted in 367 health officials (n=367). Also it was carried out a review of related literature. It was applied a questionnaire to determine the dimensions and attributes. The attributes were grouped by axis according to those with widest impact and, based on this assess, the name of the dimensions was defined taken into account various previous studies and experiences. Factor and principal component analyses were applied. Results: the 79 % of the total variance covered the following dimensions: teamwork, job recognition, working conditions and work benefits. The attributes distributed in the dimensions can certify the high level found. Conclusions: the internal customer is a key factor in any institution, and the proposed attributes for customer satisfaction and loyalty in health institutions are considered adequate. |

This means that the communication and learning of English as foreign language includes elements present in the language culture. Objective: to design a system of activities to build the intercultural competence in medical students of the Facultad de Ciencias Médicas Enrique Cabrera, Havana, Cuba. Method: a system of activities is proposed, supported by test content list, document analysis, collection and information gathered through whole research process. Several research methods were used in the study, for instance, the empirical methods (comprising the observational, survey, and interview methods), the theoretical methods (historical-logical analysis, the systemic approach and modeling). The logical thinking procedures and mathematical-statistical methods were also used. Results: learning English as an international language and its corresponding intercultural knowledge is of great value in the English subject. This learning process was supported by the application of a system of activities for the training of future doctors, who must learn to communicate in this language, which implies mastering the intercultural contents that allow them to interact efficiently with people, authorities and organizations while working in the countries where they perform their functions. Conclusions: learning English as an international language and the corresponding intercultural knowledge, through a system of activities, is essential in the training of medical students at the Facultad de Ciencias Médicas "Enrique Cabrera". |

In Ciego de Avila, Cuba, specialists in Angiology and Vascular Surgery lack professional improvement activities in general, systematic and collective participation way, straightly performed or oriented from the University of Medical Sciences. Objective: to design a professional development program for specialists in Angiology and Vascular Surgery on the treatment of peripheral vascular diseases with the use of novel techniques based on the professional problems met. Method: a qualitative research with descriptive study was carried out in Angiology and Vascular Surgery specialists linked directly to the patient care service in 2020. It worked with the whole population comprised of 19 specialists. A survey was applied and descriptive statistics were used to organize and classify the quantitative indicators obtained. Results: knowledge on how to use the novel techniques in peripheral vascular diseases treatment proved to be inadequate at a pointed level of Less adequate and Inadequate in more than 90% of studied specialists. Conclusions: the professional development program is provided with characteristics of an open system and flexibility, which facilitates its constant redesign to adapt to the specific conditions of the contexts required. |
