Likewise, a prior evaluation of the patient with risks is required by the multidisciplinary working group, including the intensivist. In harmony with the prior theme, the Cuban government assumes the challenge of ensuring the quality of maternal and child care. Objective: to characterize the quality of medical care for critical maternal patients in the ICU of the General Teaching Hospital "Dr. Agostinho Neto ", from Guantanamo, during the 2015-2016 biennium. Method: an observational, prospective and longitudinal study was carried out. The universe was made up of 202 patients admitted to the study period. The study of the quality variable of the process of medical attention to the maternal patient (PAMM) was based on the methodology of Ramos Dominguez. We have explored criteria about the structure, process and results of this medical care. Results: the quality of the attention to the serious maternal ones was evaluated as satisfactory, since the indicators of the structure, indicators of its dynamics as well as those related to the process studied were satisfactory. Conclusions: the quality of the process of medical attention to the maternal patient was satisfactory, although it is necessary to continue the management to ensure the material requirements in order to achieve excellence. |

In harmony with the above, the State has given the indication to raise the quality of the teaching-learning of the English Language to all levels and educational systems in the country. Objective: to design a system of actions in the overcoming that favors the development of communicative competence with essential character in the professional and sociohumanistic development of the health professionals. Method: a qualitative-prospective study was carried out with a universe conformed by 130 professionals of the health in the 2016-2017 study period of the Diploma in Medical Education and Sociohumanist Development. The study of the variable development of communicative competence in the English language was based on the methodology of Ronquillo Hernández E and others. The dimensions, the process for its development and the results of the actions for this competition were explored. Results: the communicative quality of the professionals increased, positions in conditions to provide better care needed by them. It was evaluated as satisfactory, both the design of the actions, dynamics and the indicators of effectiveness of the process of improvement. Conclusions: it was satisfactory given that the applied system of actions becomes a cultural technology of change in the professional dynamics in health, favoring communicative actions with people, families and communities. |

Objective: to characterize the behavior of the chronic complications of type II diabetes mellitus in elderly patients of the Medical Clinic of Family Number 5 of Caimanera municipality from September 2016 to February 2017. Method: a descriptive cross-sectional study was carried out where qualitative and quantitative analysis techniques were used. The studied population was constituted by 26 patients. Among the variables are studied the following: type of chronic complication of diabetes, age, sex, years of evolution of the disease, number of chronic complications of diabetes and level of psychological depression. Results: the most frequent chronic complication in the patients was diabetic neuropathy. Patients with fewer complications; lower age and evolutive period of diabetes mellitus complications are presented. A high percentage of patients had levels of depression between medium and high. Conclusions: the chronic complications of the DM II in the elderly patients appeared at a younger age and a shorter period of evolution, in addition to being associated with high levels of psychological depression, which makes the prognosis of the disease darker, which could increase the demand and expenses of the health services that they demand. |

Objective: to characterize the behavior of wheezing in children less than five years admitted to the respiratory service of the Pediatric Teaching Provincial Hospital "José Martí" of Sancti Spíritus, from November 2015 to November 2016. Method: a study was descriptive, transversal; the universe was constituted by 242 patients less than five years admitted for wheezing in this service. Among the studied variables: age groups, sex, family atopic background, nosological diagnosis, and phenotype. Results: predominated infants, male sex, urban residence, bronchiolitis as diagnosis at admission, the history of family atopic was more frequent among mothers with a diagnosis of asthma, the personal atopic background; persistent atopic wheezing was more frequent; Total immunoglobulin E was elevated in a frequent number of cases. Conclusions: the highest number of cases in the included patients had a risk of bronchial asthma, with predominance of males, present family history and atopic family history as well as elevated total serum immunoglobulin E. |

Objective: to characterize the current state of family violence that favors the adoption of actions in response to the need of knowledge on the part of the population and health professionals about the role of educational communication in the management of this health problem. Method: an intentional qualitative study was carried out to give a theoretical-practical answer to the problem. The study of the variable level of family violence, knowledge about the problematic that is investigated was based on Herrera Santi P, s methodology. Criteria about family violence, educational communication and its incidence in the improvement of intra familial relationships were explored. Results: the design of a system of family communicative educational actions led by professionals of the Integral General Medicine that favored the effective management of family violence as a health problem. Conclusions: the quality of the communicative family educational actions that were derived was satisfactory of this work made possible the change in the affected families with the improvement in the relations among its members as a feasible way in the achievement of the quality of life of the families. |

Objective: to describe the behavior of the suicide attempt in the Guantanamo municipality, in January-September 2016. Method: a descriptive study was carried out. The data were obtained by request from the Department of Statistics of the Provincial Health Center. The universe was 254 patients and a sample of 186 was selected through the simple random sample. The Microsoft Excel where the percentage was obtained according to the amount of data that were used to process the information, the results were presented in tables. Results: during the third trimester, the patient with the largest suicide attempt was the third one, the municipality that reported the most cases was Guantanamo, being the female with the highest incidence, ages between 25-59 years and the months of August and April respectively, the months with the highest number of patients registered were September, April and August, respectively. Conclusions: it is of vital importance to pay attention to the subject in question, giving more attention to young people, besides studying the causes of the incidences of the female sex that are adopting the position of attempting against their life. |

Objective: to characterize the behavior of knowledge about the clinical-epidemiological characteristics of leptospirosis in the students of the Hygiene and Epidemiology career of the Faculty of Medical Sciences Guantanamo. Method: a cross-sectional descriptive study was carried out during from April 2016 to March 2017. The universe consisted of 380 students and a sample of 114 was taken. Variables were studied, such as: level of knowledge about the concept, transmission routes and measures for the prevention of leptospirosis. For the development of this work, theoretical, empirical and mathematical methods were used, including historical-logical, analysis-synthesis, inductive-deductive, structural and functional systemic, document analysis, survey, specialist criteria, among others. Results: a predominance of low knowledge was found in the students surveys about the concept of leptospirosis and level of knowledge about the transmission routes, in terms of prevention measures, the general result was low knowledge. Conclusions: it was concluded that the students of the study do not have sufficient knowledge about leptospirosis, so it is necessary to carry out emergent actions to avoid this problem. |

Objective: to argue the need for knowledges on the part of the population and health professionals on the relationship between the degradation of ecosystems and increase antimicrobial resistance. Method: an intentional qualitative study was carried out to give a theoretical-practical answer to the problem. The study of the variable level of knowledge about the problem under investigation was based on the methodology of Hernandez H, Ávila F and others. Criteria were explored on ecosystems, its degradation and the results of this in the increase of antimicrobial resistance. Results: knowledges about the degradation relation in the ecosystems increases, antimicrobial resistance increase favoring the advance in the fight against this health problem, expressed in the development of consciously responsible modes of action that promote the achievement of high health education in our communities. Conclusions: it was satisfactory, the quality of health promotion actions derived from this work improved the integrality in the management of antimicrobials that favor a higher quality of life in harmony with the environment of our communities. |
| Prediction of mortality in patients with severe dengue in the Intensive Care Unit | Author : Yunieski Griñán Garrido, Yariusbi Plasencia Columbié, Argelio Díaz Ortiz | Abstract | Full Text | Abstract :Introduction: dengue is an important public health problem for endemic regions. Objective: to identify clinical, biological and imaging variables associated with the probability of dying in patients with severe dengue admitted to the intensive care unit of the General Teaching Hospital "Dr. Agostinho Neto" between January and November 2014. Method: a study was conducted epidemiological, case-control, retrospective study in which all patients were admitted to the research scenario with a diagnosis of severe dengue and whom were confirmed serologically in this studied period. Descriptive statistics and univariate, bivariate and logistic regression were used to determine the association of the selected variables with mortality. It were analyzed 90 cases, 10 deaths and 80 alive. Results: In the univariate analysis, mean blood pressure, urinary output and platelet count, glycaemia, metabolic acidosis and pericardial effusion were found to have significant differences. In the multivariate analysis, that shown an association with mortality, urinary output and mean arterial pressure, glycemia, metabolic acidosis and pericardial effusion. Pericardial effusion and metabolic acidosis have acceptable measures of diagnostic validity and safety of the probability of dying. Conclusions: Mean arterial pressure and urinary output 3, glycemia 2 and 3, metabolic acidosis with DB=8 mmol / l at time 1 and 2 and pericardial effusion at time 3 were found to be variables with predictive capacity of mortality in patients with severe dengue. |

Objective: to know the quality of life of aging adults in patients over 60 years of the University Polyclinic "Omar Ranedo Pubillones" from March 2016 to March 2017. Method: an observational, descriptive study was carried out in a sample of 121 patients selected by the simple random method. A data collection form was compiled (age, gender, comprehensive geriatric evaluation and incorporation into grandparent home). The results were represented in numbers and by hundreds, using tables. Results: men predominated, from 65 to 69 years old, married, being, also, the low incorporation to the home of grandparent in this area of health. Conclusions: it is necessary to be incorporated into the circles of grandparent where the integral evaluation is carried out, which makes the quality of life better. |

Objective: to diagnose the health of family members who have children and adolescents suffering from cancer, belonging to the "4 de Abril" University Polyclinic of the Guantanamo municipality. Method: a descriptive cross-sectional study was carried out in 9 families found from September 2013 to June 2014. The theoretical-methodological model of family health assessment was used, which includes as evaluation instruments, the perception test of the family functioning and the inventory of risk family characteristics. Results: the crossing of the results gave rise to the family health matrix. Paranormative crises were present in 66.6 % of families due to dismemberment. Only 33.3 % of the families were functional. In the socio-economic and cultural context in 77.7 % of the families, characteristics that constitute a potential risk to the health of the group were evidenced. Conclusions: the studied families presented serious affectation of family health due to a high prevalence of risk characteristics, as well as the predominance of family dysfunctionality, a situation that places them in a condition of over exertion to face daily life. |

Results: the product has a main page from which you can access the rest of the pages that make up the multimedia, through the different pre-established hyperlinks. It has videos and image galleries. Teaching support materials are offered. Conclusions: the computer product obtained was relevant and its application can be effective to generate learning with respect to the subject it addresses. |

This is evidenced by the active and unsystematic participation of the FEU leaders in the elaboration and evaluation of the educational project based on indicators, which influences the quality of the integrality process of the students. Objective: to prepare an electronic book for the evaluation of the educational project of the year, which favors the improvement of the integrality process of the students of the Faculty of Medical Sciences Guantanamo Method: an action research study was carried out in the period October 2015 to June 2016 with 18 teachers as guides, brigade chiefs and the head of the year collective of this faculty, where a methodology for the design of the electronic material was used. It consists of three spreadsheets, which allows from designed indicators to evaluate the results of the educational project, in addition, to determine the possible integral students and the fulfillment of the objectives proposed in the project. Results: the main result was the transformation of the pedagogical staff favoring a new style of work in order to raise the quality of the process and its impact on the integral formation of the students. Conclusions: it contributes to perfecting the integrality process from which it encourages the use of participatory, dynamic, democratic methods, unlike those used up to now, with old styles. |
| Intestinal tuberculosis simulating colon tumor. The patient presented symptoms compatible with intestinal tuberculosis that began with a tumor in the right iliac fossa, fever and abdominal pain. The case was assessed in the Pneumology group, where taking into account the clinical evolution and the result of the complementary examinations, it was decided to initiate specific antituberculous treatment with the diagnosis of intestinal tuberculosis. After these studies, patient, s evolution was good with the protocolized treatment. |

Fetal echography of the second trimester identified cardiovascular alterations that may have been related to Ebsteins anomaly. Diagnosis that was confirmed in the Provincial Center of Medical Genetics of Guantanamo later was confirmed in Santiago de Cuba. Despite being a rare congenital heart disease, the following were considered: the intensification of the actions in the prenatal care of pregnant women in the community, the realization and fulfillment of the prenatal programs existing in our country, mainly the prenatal ultrasounds that continue being the complementary examination of choice and offer specific genetic counseling in affected families. |

Conclusions: the chronic complications of the DM II in the elderly patients appeared at a younger age and a shorter period of evolution, in addition to being associated with high levels of psychological depression, which makes the prognosis of the disease darker, which could increase the demand and expenses of the health services that they demand. |
| Communicative competence of health professionals in the English language | Author : Karine Steyners Sánchez, Yohanna Casamayor Chacón, Yeleski Acosta Utria, Alina Samón Laffita, María Esther Guilarte Acosta | Abstract | Full Text | Abstract :Introduction: health professionals require communicative competence in the English language for their professional improvement, for their communication with professionals and patients from other nations, in addition to the internationalization of health practices. In harmony with the above, the State has given the indication to raise the quality of the teaching-learning of the English Language to all levels and educational systems in the country. Objective: to design a system of actions in the overcoming that favors the development of communicative competence with essential character in the professional and sociohumanistic development of the health professionals. Method: a qualitative-prospective study was carried out with a universe conformed by 130 professionals of the health in the 2016-2017 study period of the Diploma in Medical Education and Sociohumanist Development. The study of the variable development of communicative competence in the English language was based on the methodology of Ronquillo Hernández E and others. The dimensions, the process for its development and the results of the actions for this competition were explored. Results: the communicative quality of the professionals increased, positions in conditions to provide better care needed by them. It was evaluated as satisfactory, both the design of the actions, dynamics and the indicators of effectiveness of the process of improvement. Conclusions: it was satisfactory given that the applied system of actions becomes a cultural technology of change in the professional dynamics in health, favoring communicative actions with people, families and communities.
