Characterization of the patients with ST-segment elevation myocardial infarction, Baracoa, Guantanamo, in the period 2017-2019 | Author : Miguel Ernesto Castro-Romanoshky, EddyPaumier-Galano, Pedro LuisAbad-Loyola, Rolexis Torres-Ferrand, Viviannis Navarro-Navarro | Abstract | Full Text | Abstract :Introduction: Morbidity and mortality by myocardial infarction has not been characterized so far in the General Teaching Hospital “Dr. Octavio de la Concepcion y la Pedraja” in Baracoa, Guantanamo. Objective: Tocharacterize the morbidity and mortality by myocardial infarction on the institution in the triennium 2017-2019.
Method: A descriptive, retrospective and cross-sectional study was carried out in the patients diagnosed with infarction in the triennium 2017-2019 (n=75). Were taken into account the following variables: age, gender, clinical characteristics of infarction (painful symptoms, location, prognostic and clinical classification, complications, status of the patient at the time of discharge and cause of death). Results: 72.0 % of the patients were male, and the 37.3 % had an age ranging from 50 to 59 years old. Lethality represented a 14.7 %. The anterior left ventricle wall infarction was the most common (53.4 %). 28.0 % presented a class IV type, according to the Killip-Kimball and the Forrester classifications. 49.3 % presented high risks according to the GRACE score. 88.0 % had complications, the most common of them being the mechanical type (60.0 %). The most common cause of death was the hypoxic-ischemic encephalopathy caused by secondary atrial fibrillation (54.5 %).
Conclusions: A reference to describe the myocardial infarction in the province was elaborated.

Objective: To characterize acute kidney damage in patients with invasive mechanical ventilation in the intensive care unit at the General Teaching Hospital ¨Dr. Agostinho Neto¨ within the period 2018-2019. Method: A descriptive, retrospective and longitudinal study approved by the Ethics Committee was carried out. The study population was constituted by the total of patients with the diagnosis, according to the Acute Kidney Injury Network (AKIN). Characteristics and variables like: reason for the invasive mechanical ventilation, its duration, hemodialysis, and status of the patient at time of discharge were taken into account, along with the characteristics of the kidney injury (stages and etiology).
Results: 47.5 % of the patients treated with life support showed acute kidney injury, especially the ones with more than a week of mechanical ventilation (68.4 %). The risk of death was higher in the patients with mechanical ventilation and hemodialysis. In the patients with stage 2 and 3 of the scale used for kidney injury presented 3 times more risk compared to those who did not require these treatments. Sepsis was the main cause of acute kidney injury (69.4 %). Conclusions: the use of invasive mechanical ventilation and acute kidney injury are deeply related to each other, both of them constitute main issues in the variables of mortality.

Objective: To specify the perception of affected patients about this disease in order to support the systematization of the theoretical bases directed to the preparation in the subject of students of stomatology and comprehensive general stomatologists in training.
Method: A descriptive, prospective and cross-sectional study was carried out in the Stomatological Teaching Clinic "Julio A. Mella", a branch of the University of Medical Sciences of Guantanamo. Twenty-five patients with diagnosis of periodontal disease were surveyed to as cert a in their perception of the disease. The results were summarized by means of absolute frequencies and expressed in tables. Results: it was common that patients revealed an un favor able opinion regarding the following topics: absolute lack of knowledge about this disease (n=22), insufficient posture of the patients about gum self-care (n=22) and dissatisfaction with educational actions in terms of prevention (n=14).
Conclusions: Patients reveal an inadequate perception of periodontal disease which supports the systematization of the theoretical bases for the understanding of the need to develop in them a culture of gum care for a good periodontal health. Furthermore, the value of the stomatologist, the family, the school, the media and the community as social agents responsible for the education of the population in terms of the prevention of periodontal diseases is reiterated.

| Characterization of tuberculosis in patients from the Guantánamo province, Cuba | Author : Roxana Herrera Charro, Iván González Rodríguez, Carlos Hernández Faure, Rafael de la Cruz Vázquez, Beatriz de la Caridad Licea Sierra | Abstract | Full Text | Abstract :Introduction: The fight against tuberculosis is a social and professional responsibility, which requires its characterization, which has not been carried out in Guantánamo in the last decade. Objective: To characterize tuberculosis in patients in the Guantánamo province during the period between 2012 and 2019.
Method: The universe was made up of the total number of diagnosed patients (n = 136). The following variables were studied: age, sex, location of the disease, smear results, category at discharge, and tuberculosis risk groups. The information was obtained through the notifiable disease registries for each year and the epidemiological surveys of the outbreak controls carried out, and was summarized in absolute numbers and percentages.
Results: In 80.1% of cases, tuberculosis is in the lungs, and the diagnosis of patients with positive smear microscopy (63.2%) is more common. 76.7% of smear-positive patients are male. The most common risk groups for tuberculosis are: smokers (26.4%), immunosuppressed (21.6%) and alcoholism (19.1%).
Conclusions: In Guantánamo province, pulmonary localization tuberculosis and patients with positive bacteriology prevail. Those affected are mainly men, they are between 45 and 54 years old and have recently been diagnosed. The disease affects more in those with a history of being smokers, immunosuppressed and alcoholics.

Objective: To describe the clinical-epidemiological characteristics of the patients with soft tissue infections hospitalized in the Pediatric Teaching Hospital “Hermanos Cordové”.
Method: An observational, descriptive, cross-sectional study was carried out in the services of pediatric clinic in the Pediatric Teaching Hospital “Hermanos Cordové”, in Manzanillo, Cuba, during the year 2018. All discharged patients who had a confirmed diagnosis of this type of infection were studied (n=298). The following variables were taken into account: anatomical location, classifications of the different types of skin and soft tissue infections, treatment used and hospitalization.
Results: Male gender prevailed in the patients (57.7 %), and also the ages ranging between 10 to 14 years old (34.9 %). Infections located on the limbs were described in 113 patients (37.9 %). Impetigo was the most common diagnosis (42.3 %), and the most frequently used antibiotic was cefazolin (77.2 %). Most of the patients were hospitalized up to 7 days (293), with a satisfactory evolution of the disease.
Conclusions: Patients with soft tissue infections in the services of pediatric clinic in the Pediatric Teaching Hospital “Hermanos Cordové” are mainly characterized for being male, especially from ages raging between 10 to 14 years old, with impetigo being the most common infection, mostly located on the limbs. Cefalozin resulted very effective for the treatment of these infections, making possible the hospitalization time to be lesser than a week.

Objective: to evaluate the therapeutic results of the application of botulinum toxin type A in patients with hemifacial spasm treated in a Neurology consultation. Method: a descriptive and longitudinal study was carried out in 21 patients attended in Neurology service with hemifacial spasm and botulinum toxin type A treatment, fromJanuary 2018 to January 2019. The variables studied were: age, gender, hemiface affected, latency period, duration, disappearance of the clinical effects of botulinum toxin type A, and percentage of functional disability in patients when applying the disability scale before and after the treatment.
Results: Female cases predominated (57.1%), agesover 55 years (57.14 %), the most common affected side of the face was the left, with 57.14 %, the latency period of botulinum toxin type A was between the third to the tenth day in 14 of the patients (66.67 %), the maximum time of duration of treatment ranged from one to two months (57.14 %), and the disappearance of the therapeutic effect of the toxin in the patients was between the third to the fourth month (52.38%). After the application of the clinical assessment scale of hemifacial spasm, a decrease in the percentage of patients with functional disability was evidenced.
Conclusions: Botulinum toxin type A is effective in the treatment of hemifacial spasm.

Objective: To create an educational multimedia software about neoplasms of the cervicofacial region, in order to support the learning process of the stomatology student.
Method: A technological development study at the University of Medical Sciences in Granma was carried out, from September 2018 to June 2019. In order to create the multimedia software, different working stages were specified: search and gathering of information, selection of tools, and design. The final product was validated by experts’ criteria. The effectiveness and impact of the software in the learning process was evaluated through a pedagogical quiz to all of the students who were involved in the study.
Results: The multimedia software had a clear and attractive design, offer ingan open exploration environment, and a fast and easy method to get to the information needed. Experts evaluated the product as “very appropriate” in its entirety, and the users’ rating on the points “Ease”, “Functionality” and “Originality” were “good. When applying the Mc Nemar Test, all values were significant; demonstrating the effectiveness of the software developed.
Conclusions: This multimedia software allows the students of Stomatology to geth old of the information on neoplasms of the cervicofacial region, making ituseful as support material for teaching.

| Epidermoid carcinoma on the conjunctiva of the eye. The eye examination showed the presence of conjunctival hyperemia, a highly vascularized conjunctival lesion in the internal angle of the right eye that invaded the cornea. Exceresis and biopsy were performed which reported an epidermoid carcinoma on the conjunctiva of the eye. Epidermoid carcinoma on the conjunctiva of the eye is a rare cancer, but upon clinical suspicion, an adequate clinical semiology must be performed to obtain the definitive diagnosis and apply the treatment according to the protocols of action. |
