Abstract :Gastric neuroendocrine carcinomas are rare and have a poor prognosis. The present case concerns with a 55 year old female who presented with complaints of recurrent vomiting on and off, hematemesis and weight loss and history of lumbar stenosis. Esophagogastroduedenostomy (EGD) showed a large ulcerated growth in the antrum. Computed tomography abdomen revealed an ill defined soft tissue density in the gastric antrum, a partial gastrectomy was performed. Microscopic evaluation revealed a neuroendocrine neoplasm. Immunohistochemically positive for Chromogranin A and Non Specific Enolase (NSE). A diagnosis of Neuroendocrine carcinoma of the stomach was given based on recent WHO classification of Neuroendocrine carcinoma of the stomach and on mitotic index with reference to grading scale.

Abstract :Background : During the conference of ASCO 2015, there was no consensus about TIL role in beast cancer and no recommendations concerning the microscopic assessment of TIL. In fact, the patients could be put on anti-PD1 drugs without the necessity of highlighting PD1 positive cells by using immunohistochemistry. Nevertheless, many questions about the prognostic impact of TIL, the methods of assessment, the type of TIL to count remain unresolved. Material and Methods : We performed a review of the literature on the sites : Pubmed and Cochrane. We used the key-words : �TIL in cancer�; �TIL in breast cancer� and �prognostic impact of TIL in breast cancer�. Results : According to our inclusion and exclusion criteria, thirty eight articles were retained. Our review of the literature showed that assessing TIL in high grade tumors seem unnecessary. They seem available in intermediate graded tumors. In neo-adjuvant and adjuvant conditions, CD3+ lymphocytes seem to be correlated to a good response to chemotherapy. After a chemotherapy, quantification T reg lymphocytes CD4 + FOXP3+ seems helpful because the decrease of their number is correlated to a good prognosis. Conclusion : The role of TIL in breast cancer is clearly established. The mechanisms of immune escape induced to discovery of immune therapy. The role of the microscopic examination and the subtyping of TIL using immunohistochemistry hasn�t been clearly established. Through this review of the literature, we tried to establish a diagram highlighting the different subtypes of TIL to evaluate and their prognostic impact.

Trichomonas Vaginalis among Parturient Sudanese women in Kassala Hospita, Eastern Sudan |

Abstract :Trichomoniasis in the women is usually asymptotic however the disease might be manifested as vaginitis, cervicitis, urethritis, pelvic inflammatory disease (PID), and adverse birth outcomes. Methods: A case-control hospital based study conducted at Kassala Hospitals, eastern Sudan during the period from 1st January 2015 to 30th June 2015 to investigate the prevalence rate of Trichomoniasis during pregnancy and its impact on neonatal outcome. Results: During the study period there were 199 infected women with T vaginalis among 2374 deliveries yielding a prevalence rate of 8.3%. The vast majority (140/199, 70.4%) was asymptomatic while the rest presented with vaginal discharge (33/199, 16.6%), itching (16/199, 8%) and dysuria (10/199, 5%). With regard to membranes status and neonatal outcome higher proportion of infected women presented with premature ruptured membranes (30, 15.1% Vs 6, 3%; P = 0.000) and gave preterm birth (31, 15.6% Vs 7, 3.5%; P= 0.000). Using logistic regression analysis the study showed significant association between Trichomoniasis, preterm birth (CI= 1.1 � 13.6, OR= 3.9, P= 0.030) and premature rupture of the amniotic sac before 4 centimeter dilatation (CI= 1.0 3.2, OR= 1.8, P= 0.025). Conclusion: Trichomoniasis is highly prevalent among parturient women in eastern Sudan, and there is significant association between Trichomoniasis, preterm birth and premature ruptured membranes.

Abstract :A week old male infant presented with severe tenderness all over the body with associated continuous crying. There was bowing of both limbs. A total body radiograph revealed multiple healed and healing fractures in the long bones and ribs. The long bones were short, broad and bowing with thinning of the cortex. The spine shows compressed, flattened and irregular vertebral bodies. Some infants� deaths has been recorded in the family. Osteogenesis imperfecta is a rare disorder of connective tissue which is characterized by skeletal deformity, bone fragility, fractures, ligament laxity, hearing loss, blue sclera, dental abnormality and thin skin. All those features will not manifest in one case; they are therefore group into four types and subtypes. This case is more of Type 2 and Type 2A Osteogenesis imperfecta.

Tuberculosis of the Talus in a Child - A Rare Entity |

Abstract :A 10 year old boy presented with fever, local swelling around the ankle with tenderness and antalgic gait, without any antecedent trauma or discharging sinus. He had no evidence of pulmonary tuberculosis. Laboratory tests revealed signs of inflammation with normal Mantoux test and chest X-ray. An irregular lytic lesion of the talus was seen on the x-ray of the affected part. Ziehl Nelson staining of the aspirated fluid revealed acid-fast bacilli. Material obtained after curettage and bone grafting confirmed the diagnosis of tuberculosis. He was treated conservatively with splintage and 4 drug anti-tubercular chemotherapy for 18 months, without any residual tenderness or foot deformity at the final follow-up. This case has been reported because of its rarity and atypical clinical presentation.

Abstract :This research identifies the important determinants of child mortality from 2012-2013 G.C in Arba Minch hospital. Logistic regression method was used to determine the impact of child mortality. The results show that among bio-demographic factors, gender of child, mother age and breast feeding are the important determinants for child mortality. However, breast feeding has a significant impact on child mortality. Among Socioeconomic determinants education, household size are the most important determinants for child mortality. Therefore policies should be revised and implement and health intervention program that focus on mothers and children health should be strong to achieve the Millennium Development Goals of child mortality in the remaining years.
