Abstract :Health always remains a central component of
human social structure and considered as a major
instrument of overall socio-economic
development. Concept of health has been defined
and understood in different ways by different
cultures. The two major division of medicine
namely curative and preventive medicine came
into light at the close of 19th century. Although
origin of Medicine and Public health goes back to
ancient Greek mythology, Asclepius was the
Greek god of Healing. He was the son of Apollo
and the Coronis. Asclepius was married to Epione,
with whom he had five daughters: Hygieia,
Panacea, Aceso, Iaso, and Aglaea. Hygieia was the
goddess of health, cleanliness and hygiene1
.
Follower of panacea practiced curative medicine
and were called healer and followers of Hygieia
were called as Hygienist or Public Health experts. |

Deceased Donation: Evacuating Assets of a House on Fire |

Abstract :Scribbling the narratives of a doctor who bemoaned her inability to keep her
mother alive through organ donation, this article takes us to a gamut of
nuances of organ donation from deceased donors. This article traces the
history of organ donation from deceased donors and briefly discusses a few
milestones that were achieved for smoothing organ donation; the first
milestone being the invention of a new term coma depasse (a state beyond
coma) by two French physicians in 1959; second being the invention of a new
medico legal definition that is brain death in 1968 and third being the
introduction of immunosuppressant in the form of cyclosporine in 1980.
While discussing two forms of deceased donation i.e., non-heart beat donation
and heart beat donation, the concept of organ donation from deceased donors
is equated with evacuating assets of a house that is on fire. This article focuses
attention on a number of people centric challenges like increasing demand of
organs, restricted acceptance of brain stem death, lack of awareness, lack of
incentives and follow-up care of the recipients. A gamut of program centric
challenges like establishing national organ donor registry, management of
donors, uniting organizations through electronic linkages for effective organ
allocation and sharing, evolving organ allocation and sharing mechanism,
synchronizing team efforts for packaging, transportation and transplantation
of retrieved organs besides establishing regular auditing system of retrieved
organs are briefly described. This article stresses the need for uniform
international guidelines for donation after cardiac and brain stem death. The
article recommends introducing the concept of brain stem death and organ
donation in the curricula of all the professional trainees taking care of
patients and introducing a chapter in school curriculum for fostering the
cause of saving lives through organ donation from brain stem dead donors in
our country. |

Pay for Performance Programs in Healthcare: An Analysis of Findings from District Mewat in Haryana State of India |

Abstract :Pay for performance has emerged as an expedient for popularizing health
programs among the people as also for motivating the healthcare providers/
health workers to implement these programs efficiently and effectively.
Janani Suraksha Yojna (JSY) is a safe motherhood scheme of the
Government of India to encourage institutional deliveries in the country with
the objective of bringing down maternal and infant mortality. The
Government of Haryana further introduced Jacha- bacha (mother and child)
scheme in the state in 2008 to further encourage institutional deliveries in the
state. This article analyzes the findings regarding implementation of JSY and
Jacha- bacha schemes in a sub- center in district Mewat, which is the most
backward district of Haryana, located adjacent to Delhi. The analysis draws
upon extensive literature on theories of motivation and experience of
implementation of pay for performance schemes in different parts of the
world |

Prevalence of Dysmenorrhea among Adolescent Girls: A Cross-sectional Study from Urban Central India |

Abstract :Adolescence is a transitional journey from childhood to adult life along with
pubertal development and sexual maturation. One of the major physiological
changes that take place in adolescent girls is the onset of menarche. To add to
their worry, problems of irregular menstruation, excessive bleeding and
dysmenorrhea are there. The true incidence and prevalence of dysmenorrhea
are not clearly established in India. Our study aimed to assess the prevalence
of dysmenorrhea among urban adolescents. A cross- sectional study was
conducted from April to September 2013 among all girls of selected
government and non government higher secondary schools of Raipur city
using a pre designed, pre tested interview schedule. Data were entered in
computer and analyzed using SPSSvs14. Descriptive statistics were used to
analyze the result. A total of 577 female students were enrolled. Prevalence of
dysmenorrhea in our study was 71.4 %. Dysmenorrhea occurred in students
with both regular and irregular cycles. The findings of this study thus indicate
the enormity of the problem and the need for appropriate intervention among
these groups |

Factors associated with Exclusive Breast Feeding (EBF) and Complementary Feeding in an Indian Urban Community: A Cross Sectional Study |

Abstract :Context: Inappropriate feeding practices are very much prevalent in India in
areas where there is a high cultural influence on dietary habits, which starts
right from the birth and remains throughout the life.
Aims: To study the infant and young feeding practices and factors affecting
the exclusive breast feeding in children aged 6-23m.
Methods and Material: A community based cross sectional study was
conducted during the period Jan 2008 - Dec 2008 in Mehrauli (New Delhi).
Feeding practices in the community was assessed using a semi structured
questionnaire from the mother/ guardian of 309 children in the age group of
6-23m.
Results: Breast feeding was initiated within an hour of the birth in only 10.7%
(33) of children. Around half (153) of the children were fed with prelacteals
and colostrum was discarded in two-thirds (208) of the total. Exclusive breast
feeding was dismally low with only 6.8% (21) mothers having practiced it.
EBF was found more in institutional delivered and normal birth weight
babies in comparison to domiciliary delivered and low birth weight babies
respectively (p<0.05). Only 31.7% (98) children were given complementary
feeding at the appropriate age, and feeding in terms of calories was
inadequate in three-fourth (232) of the total subjects. Knowledge assessed
about IYCF (Infant and Young child Feeding) was seemingly poor amongst
the mothers in the community.
Conclusions: Feeding practices are considerably errant in the community due
to the widely prevalent myths and rituals. There is a need of consistent reenforcement
of IEC regarding IYCF by the health workers and AWWs to
dispel the misconception from the community to improve the health status of
the children |

Tartrazine Induced Fixed Drug Eruption |

Abstract :A 29 year old oncology resident presented to a
dermatologist with the complaints of burning pain
and hyperpigmentation of lips of acute onset. He
was a non smoker without any significant history
of known drug allergy or symptoms suggestive of
intestinal polyposis or any endocrinological
disorders. He was not exposed to any local
irritants in the form of chewing tobacco, its related
products and other chemicals. Detailed history
revealed that he had suffered from 3 episodes of
diarrhea one day back, took a tablet containing
combination of the drugs norfloxacin and
tinidazole with tartrazine as a coloring agent. He
felt burning sensation in upper and lower lips
seven hours after the drug intake which increased
in severity at night. |
