| INTRAUTERINE INSEMINATION OUTCOME USING TOW SPERM PREPARATION TECHNIQUES CENTRIFUGATION SWIM-UP AND DENSITY GRADIENT TECHNIQUES | Author : Dr. Sahar Muhssin Muhammad M.B.Ch.B,Dr.Samira A Muhsin M.B.Ch.B, | Abstract | Full Text | Abstract :Background: Artificial insemination (AI) is a procedure in which sperm are placed in the cervix or high in the uterine cavity through a transcervical catheter. The aim is to deposit sperm as close to the oocyte as possible (ASRM, 2006). It may be preceded by ovarian stimulation with gonadotropins or clomiphene to encourage multiple oocytes development, especially in cases of unexplained infertility (Mathuret al., 2007).Sperm separation procedures are able to significantly improve the sperm quality, such selection of spermatozoa separates motile sperm from nonmotile, removes seminal plasma, and infectious agents, other background materials and debris (Zavos, 1992) and also in the same time initiates the capacitation of sperm (Henkel and Schill, 2003). Patients, materials and methods: One hundred thirty-five infertile couples were participated in this study where the male partners needed in vitro sperm preparation and activation, while the female partners were normal and subjected to ovarian stimulation program. In vitro sperm activation was performed using two different techniques, the centrifugation swim-up technique with Global medium and density gradient technique with ALLGrad solution and Global medium. Randomly the infertile males shared in this study were grouped into two groups according to type of technique used in sperm preparation. In the first group the centrifugation swim-up technique was applied for 70 infertile males, while in the other group 65 infertile males have density gradient technique for their in vitro sperm activation. Both groups were subjected to IUI. Results: Intrauterine insemination was applied for the wives of all infertile males involved in this study after having in vitro preparation for their sperm samples, among the 135 couples involved in the present study, 16 couples have positive IUI outcome, 9 couples were in the group used density gradient technique with percentage of (13.8%) which is higher than the percentage of positive IUI outcome in the infertile couples used centrifugation swim-up method where it was (10%) and count of 7 from 70 patients used that technique. Conclusions: Density gradient techniques with ALLGrad solution has better IUI outcome compared to centrifugation swim-up technique. An improvement of percentages of sperm progressive motility, grade (A) progressive sperm motility, normal sperm morphology that were higher in density gradient technique were the main factors that enhance the IUI outcome. |
| AYURVEDA AND MENTAL HEALTH: AN INSIGHT | Author : Gaurav Phull,Rekha Phull,Dimple Aggarwal | Abstract | Full Text | Abstract :Ayurveda is a science which is regaining its worth in 21st century. There are number of psychotic &somatic diseases which were widely explained in samhitas many centuries ago and the role of manas is also explained thoroughly. In brihtrayi, Charaksamhita explained the manas, its types, Mansika gunas, mano-vikara as well as chikitsa in a very descriptive way. The combination of body, senses, mind and soul, results in existence of living being. Manas has been labeled to play an important role in causing physical diseases like grahni, ardhavbhedaka, atisara, chhardi etc. The definition of health given in Ayurveda is comprehensive which included mental wellness as an important component. In modern science, the mental health and psycho-somatic diseases were introduced in 1908. The rich heritage of Ayurveda is depicted by the fact that even WHO reformed their definition of health few decades ago by introducing the mental health as the important aspect of health. Charaksamhitaadvocates many preventive measures to save from various mental disorders- Sadvritta, Achararasyana, Ritucharya and Dincharya are some of them. Simultaneously, various aushadhis are also recommended. Thus it is clearly evident that mental health and disorders related to it were well addressed in Ayurveda and if principles explained in ancient texts are followed properly, many diseases of present time can be prevented. This can help in achieving health in real sense. |

In approximately 40% of infertile couples, the male partner is either the sole or a contributing cause of infertility. A severe defect of spermatogenesis including reduced sperm count, motility and many sperm with abnormal morphology, will probably always be relevant. However, mild defects may only become relevant as the age and the duration of infertility increases. Objectives: Evaluate the relationship of age and duration of infertility with sperm parameters and correlate them with IUI outcome. Patients, materials and methods: One hundred thirty-five infertile couples were participated in this study where the male partners subjected to in vitro sperm preparation and activation, while the female partners were normal and had ovarian stimulation program. In vitro sperm activation was performed using two different methods, the centrifugation swim-up technique and density gradient technique with. The mean ages of infertile males were 35.614±0.539 years with a range from 22 to 52 and the mean duration of infertility was 6.113±3.75 years with a range from 1 to 20 years. Both groups were subjected to IUI. Results: Lowest sperm concentration and motility with age group (> 50) years. However; best results in this study regarding total sperm motility was with duration of infertility (3-4) years and the lowest total sperm motility with duration (>10) years. Regarding IUI, the highest IUI outcome was with age group (30-39) years and with duration of infertility (<2) years (25%). Conclusion: This study concluded that sperm parameters and IUI outcome inversely correlated with duration of infertility. Also better sperm parameters regarding concentration, motility, morphology and IUI outcome was with young age group. Since the sperm parameters and IUI outcome decline with age and with longer duration of infertility as shown by the results of this study so better to interfere with IUI in couples with duration of infertility less than two years. |
