| Has Regular Heart Rhythm A Positive Influence on Nightly Desaturations? The hypoxic trigger due to repetitive nightly apnoic events may be responsible of the onset and the maintenance of the arrhythymias such as atrial fibrillation or atrial flutter. The evidence supports a close link between these two pathological entities. Even if the perspective was always directed from sleep apnea to cardiovascular disease, the physiopathological pathways that link both are not still completely know. So it is thinkable that also the cardiovascular condition might influence the respiratory trend as the first input regarding the continuous changes happening during alternance between arrhythymias and restore heart rate stability. This case report could be an opportunity of reflection in this way. Future research is expected to clarify the real link between heart and sleep in the different cardiac condition and vice versa. |

and stress. From this result, a method to eliminate stress and favorably influence blood vessels and activity capacity was
examined. We developed a method where elderly participants danced to familiar music while executing brain training tasks.
Methods: Verified method is a brain training developed from music therapy and dance. As brain training, the effectiveness
of training tasks such as dual-task (performing two tasks simultaneously) and n-back task (recall task for items shown n
steps earlier) have been verified. Additionally, methods developed in this study utilized "ostinato", a rendition often used in
music such as folk music. This method repeats a short phrase multiple time. As it has a trait which makes it easy to retain
and recall, a repeated melody which speeds up is easily retained in memory. Analysis in that is a comparison between
intervention and control. Cognitive tests and stress checks were compared with a paired One-way analysis of variance.
Result: Data of 102 people were analyzed. In comparison to the control group, cognitive function was improved and distress
was reduced in the intervention group. Scores of cognitive test items: immediate memory and recall delayed memory were
significantly improved (p < 0.05).
Conclusion: The brain training dance to familiar music improved cognitive abilities. This activity can relieve stress.

volume of fun activities available to the elderly |
| Concurrent Pulmonary Embolism with Paradoxical Embolic Stroke and Renal Infarction in The Setting of Patent Foramen Ovale and HIV-Related Antiphospholipid Syndrome: Case Report and Review of The Literature | Author : Alexander Andreev MD1, Farhana Begum2, Mohammed Zaman MD3, Junior Kalambay MD4* | Abstract | Full Text | Abstract :Background: HIV is commonly associated with positive antiphospholipid antibodies and lupus anticoagulant (LA) assay,
but association with clinical evidence of thrombosis is not well established. To our knowledge, we are the first to describe
concurrent Pulmonary Embolism (PE), ischemic stroke due to paradoxical embolism, and renal infarction occurring in the
presence of Patent Foramen Ovale (PFO) and HIV-related Antiphospholipid syndrome (APS).
Clinical Presentation: This article reports a 60-year-old male with pertinent past medical history of HIV presented to the
ER with left-sided weakness, slurred speech and left facial droop. Patient was highly suspected of ischemic stroke and given
tPA. The CT angiogram (CTA) head and neck was negative, but revealed bilateral PE, later confirmed by CTA chest. This
was determined to be an unprovoked PE. CTA chest also revealed large left renal infarction, later confirmed by renal
radioisotope scan. Repeated EKG showed atrial flutter, and TTE with bubble study revealed a 3 mm PFO. Hypercoagulability
work-up revealed LA.
Conclusion: Clinical significance of aPL antibodies, positive LA assay in HIV infection remains unclear. Yet, several casereports, including our own, describe major thrombotic events, including PE, CVA and renal infarction. We can conclude, that
screening for APS in HIV patients, especially those with intracardiac shunts, followed by proper anticoagulation, may
prevent life-threatening thrombotic events. |

We report a
complex case of multiple Pancreaticocolonic fistulas that was successfully treated with extended left hemicolectomy. Case
report A forty-three-years-old male patient, presented in our emergency department with epigastric pain, vomiting,
diarrhea and weight loss for 4 weeks duration, with past history of acute gall stone pancreatitis 10 weeks earlier. Contrast
enhanced computed tomography abdomen showed multiple Walled-off Pancreatic Necrosis in the peripancreatic, right
paracolic and left paracolic regions up to left inguinal region with extensive Pneumoretroperitoneum. The periduodenal
collection caused duodenal compression. Laparotomy done for pancreatic necrosectomy, relieving the duodenal
compression, and drainage of all collections. We noticed multiple Pancreaticocolonic fistulas, 5 in numbers, between the
transverse and descending colon and their neighboring collections, extended left hemicolectomy done. Multiple abdominal
drains had been put for continuous postoperative irrigation. The patient discharged home but after 3 months of a hectic
post-operative course. Discussion To the best of our knowledge this is the first case to be reported in the literature with
multiple walled-off pancreatic necrosis associated with multiple Pancreaticocolonic fistulas (5 in Number) in both
transverse and descending colon. Conclusion Necrotizing pancreatitis is a devastating disease, the presence of
Pneumoretroperitoneum does not essentially only point to infected necrosis, but the possibility of Pancreaticocolonic fistula
should always be kept in mind and searched for the diagnosis of pancreaticocolonic fistulas.

| Complicated Pancreatitis: A Rare Complication Following Scoliosis Correction Surgery | Author : Barbara Dolaszynska* | Abstract | Full Text | Abstract :Acute and chronic pancreatitis post scoliosis correction surgeries have been reported before as a rare complication. Many
hypothesis were suggested including direct trauma and waves generated by the high speed drilling and re-allignment of the
spine after the surgery. In all the cases reported before there was an element suggesting those hypothesis. In this case, there
was no evidence that the pancreatitis is caused by the spinal surgery at all. 14-year-old girl who was admitted for elective
scoliosis surgery on. She was unwell post operatively, with a rising CRP and an increasingly distended abdomen. A CT abdomen
was performed and showed some free fluid but no free gas. She went for a diagnostic laparoscopy due to failure of conservative
management. This showed extensive inflammation of the right side of the colon, adherent to the bowel and that led to laparotomy
and extended right hemicolectomy and ileostomy formation. The surgical samples from the bowels has been sent for pathology
in which showed manifestations of Pancreatitis. No cause identified for diagnosis. The histology Macroscopy report of the
biopsy samples in this patient showed congested and oedematous serosa with adhesions and patchy, fibrinopurulent exudate
extending along almost the entire length of the specimen (Right Hemicolectomy). Microscopically: There is extensive mesenteric
panniculitis with multifocal neutrophilic abscesses surrounding fat necrosis. There was extensive saponification and necrosis.
There are acute and chronic inflammatory cells within the fat necrosis -foamy histiocytes, lymphocytes, neutrophils and plasma
cells.

| Dyeing Effect of Brassica Oleracea Extract in Onion Roots and Human Blood Cells | Author : Nilgün Güler Kusçulu1 | Abstract | Full Text | Abstract :Environmentally friendly natural dyes have been used in food, textile, medicine fields. The red cabbage (Brassica oleracea)
plant including cyanidin is usually used as food material. However, due to the presence of anthocyanins, cyanidin can be
evaluated as a source of dyes. The cells were examined by staining. Onion roots and human blood used to disseminate the
use of plant-derived dyes and to show the staining of the nuclei of different cells. The alum also used as a mordant material
for bonding between cell core and dyestuff. Furthermore, only onion roots treated with salicylaldehyde for been having cell
walls and then taken to the dye solution. A few drops of blood taken from healthy, voluntary 20-years old person spread
peripherally. A top of onion provided from a local market immersed in a small beaker of water and its roots put into 70%
ethyl alcohol. 20% red cabbage extract prepared. 1 piece of onion root and blood preparation put into the extract. They
subjected firstly at room temperature and secondly 50° C for one-hour to dyeing processes. In addtion, one of the onion root
immersed in the dye solution after standing in salicylaldehyde. The other root directly added to the dye solution. Both
solutions kept at 50° C for 1 hour. After staining, onion root cells and blood cells washed. They examined as × 40, × 100
magnitudes in the light microscope. No staining observed in both cell types at room temperature. When the temperature of
the dye bath increased, a blue coloring observed in the nucleus of onion stem cells. Especially erythrocytes in blood cells
stained dark blue with alum. No staining observed in the onion stem cells treated with salicylaldehyde. That is, the onion
roots stained in the dye bath containing alum mordant without salicylaldehyde. |

Due to high circulating insulin levels in the blood, patients will often, if not most times, present with neurological symptoms. Symptoms can range in severity depending on the degree of hypoglycemia. In the most severe form, patients can experience seizures and coma. In less severe cases, they can present with fatigue, confusion, weakness, change in personality. Symptoms that resolve with food intake is a traditional hallmark. This hallmark diagnostic criteria should alert the health care provider to this diagnosis when a patient presents as such. |

We describe a 56-year-old post-menopausal woman who presented with a huge, right breast mass extending from the clavicle to the mid abdomen. A chest-abdomen-pelvis computed tomography showed the mass invading the pectoralis muscles and right axillary lymph node with no evidence of distant metastasis. A needle core biopsy revealed an infiltrating ductal carcinoma (Scarff-Bloom- Richardson grade 3). A multidisciplinary collaboration was undertaken, and the patient was started on docetaxel; however, after three cycles of docetaxel, she showed no improvement and was readmitted for palliative mastectomy with skin grafting. A post-operative histopathology examination of the excised tumor revealed an invasive micropapillary carcinoma. A multidisciplinary approach is the cornerstone for the treatment of LABC. In the case of chemoresistance, very few treatment options are available. |
