Abstract :Involvement of the sacrum is extremely rare, and it has also been reported that the diagnosis of osteoid osteoma in the sacrum can be delayed compared to other skeletal locations. We report the case of a six-year-old girl student who complained of chronic back pain for several months and with some relief with non-steroid anti-inflammatory drugs (NSAID). Further investigations, which included bone and CT scan, revealed the presence of an osteoid osteoma at the second sacral vertebra (S2 vertebra). The patient made an excellent recovery after surgical excision. |

Abstract :The congenital luxation of the knee is an unfrequent affection, existing many theories about its appearance. In some cases, it is considered an isolated deformation although it is relatively frequent to associate it to other congenital malformations, such as the dysplasia on the development of the hip, feet deformities or dislocation of the elbow. The treatment of this kind of affection depends on the seriousness and flexibility of the fondness, having like therapeutic lines the placing of serial casts, by using Pavliks armor or plastia of the four headed muscle on severe cases. We present a case of congenital luxation of the knee treated by the use of serial casts, with a favorable evolution. |

Abstract :Coronavirus caused an epidemic in China in December 2019 at an animal market where live and dead animals were sold in Wuhan, China. In a short time, this epidemic spread to different continents.
This virus has been called the 2019 new coronavirus (2019-nCoV) by the World Health Organization. Unlike both MERS-CoV and SARS-CoV, Covid-19 is the seventh member of the coronavirus family that infects humans. It is characteristic for Covid-19 pneumonia that there are subpleural localized ground glass opacities and numerous irregular areas of consolidation in both lungs and especially in the lower lobes.In this case study, we aimed to present a Covid-19 positive in 58 years old man patient with cough, high fever, beginning breathlessness and chest pain on the left, accompanied by pneumothorax in the left hemithorax, after covid -19 diagnosis.
Radiographic imaging of the patient revealed pneumothorax on the left and ground-glass opacities in the bilateral lower lobes. The patient underwent tube thoracostomy from the left hemithorax lateral.
It should be kept in mind that in patients with Covid-19 pneumonia, it may develop in pneumotoacies secondary to lung parenchymal damage. Mortality rates can be reduced in patients with early diagnosis and treatment. |

Results of the application of neurovascular homodigital flaps on the cutaneous defects of the digital fingertip |

Abstract :Introduction: the traumatic injuries of the hand are in the top of the list of industrial working accidents. The treatment of these injuries has as goal to restore the function of the fingertip, improving the life quality of the patients.
Objective: to evaluate results of the application of neurovascular homodigital flaps on the cutaneous defects of the digital fingertip of the large fingers of the hand.
Method: it was perform an observational and descriptive type investigation, in a case chain of 33 patients on the Hermanos Ameijeiras Hospital in the period of October 2013 up to January 2015, were it was apply neurovascular homodigital flaps with direct circulation (17 patients) or indirect circulation (16 patients).
Results: male sex was more affected, with a higher frequency between 36 and 41 years old and also over 41 years old. There was confirmed 4 complications representing the 12%. It was obtained a 72.7% of good and excellent results with the application of these surgical techniques getting an appropriate protector sensibility and a satisfactory cutaneous covering. The patients were satisfied in a 93.9% of the cases with the application of these procedures.
Conclusions: it was demonstrated the efficiency of the application of neurovascular homodigital flaps on the cutaneous defects of the digital fingertip of the large fingers of the hand. |

It Just Takes One Phone Call: Simultaneous Activation to Reduce Referral Times |

Abstract :When there are maternal and neonatal emergencies in the Emergency Department, Emergency Physicians will manage them and will call on the Obstetrics and Gynaecology as well as Neonatalogy doctors and staff as needed. Thus far, these staff have to called individually and it takes time to repeat the history and relevant information to each one of them. We embarked on an initiative to have simultaneous activation of the relevant personnel once such emergencies surfaced. This would be through an agreed upon model and will be conducted via the operator and institution’s switchboard. All the relevant staff involved in the activation will get the information straight away, simultaneously and immediately upon activation.
To be able to do this, we organized several meetings and brainstorming sessions with the disciplines involved, tried out insitu simulation scenarios with failure mode effect analysis, as well as applied the PDSA cycle (Plan, Do, Study and Act) to the initiative. After the robust testing and discussions, which took some 6 months, the final pathway and algorithm for simultaneous activation and management was executed. |
